Christian Care Communities is seeking a full-time Social Services/Admissions for our Christian Health Center located in Hopkinsville, Kentucky.

Participates in community planning related to the interests of the Facility and the services and needs of the resident and family.

Develops and submits to the Administrator a written plan of correction for social services and admissions deficiencies noted by appropriate authorities.

Participates in discharge planning, development and implementation of social care plans and resident assessments.

Develops and manages preliminary and comprehensive written social service plans of care for each resident, identifying problems/needs and goals of the resident.

Provides information to resident/families on Medicare, Medicaid and other financial assistance/social service programs available to resident.

Identifies the need for and obtains resources from community social, health and welfare agencies to meet the needs of the resident.

Interprets social, psychological and emotional needs of the resident/family to appropriate staff and attending physician.

Coordinates and performs all resident admissions procedures, and assures proper completion of required forms and reports obtaining name, address, age, religion, persons to notify in case of emergency, attending Physician, and individual or insurance company responsible for payment of bills.

Explains facility regulations such as visiting hours, payment of accounts and schedule of charges and obtains signed statement from patient to protect facility's interest.

Enters patient admitting information into computer and routes printed copy to designated department. Compiles data for occupancy and census records.

Arranges transportation to outside events/activities when necessary.

Runs errands, reading and securing material for residents with sensory and hearing defiencies.

Encourages resdients’ religious participation, coordinates needs with Chaplain.

Qualifications:Bachelor’s degree in Social Work or other related degree from four-year college or university, must have at least two years social program experience, one of which was full-time in a patient activities program in a health-care setting; or be a qualified occupational therapist or assistant, or have completed the advanced training course approved by the state.
