Christian Care Communities is seeking a full-time Service Coordinator for our Chapel House located in Hopkinsville, Kentucky.
Under general supervision, responsible for the service management function within assigned facility. Conducts all job responsibilities in compliance with all policies and procedures of Christian Care Communities and within the parameters of the Organization’s mission, vision, and values.
Develops contacts with service providers and agencies for resident referrals and ongoing service management responsibility. Meets with providers as necessary.
Educates residents, families and staff on available community resources, and acts as liaison between community agencies, service providers and residents.
Assists and advises residents and families of the services which may be necessary to maintain a self-reliant lifestyle. Assists management in identifying residents who need assistance.
Advocates and/or negotiates with service providers on behalf of residents. Monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory.
Promotes wellness activities for all residents. Encourages residents to be proactive in meeting their social, psychological, and physical needs.
Assists residents in building informal support networks among themselves and with family members.
Performs administrative recordkeeping of all activities to include: documentation of contact with residents, providers and families; maintenance of resident files including services provided and resident/family meeting notes, completion of reports as directed by management or to comply with governing regulations.

| Qualifications: | High school diploma or GED required; 4 year college degree with concentration in Social Work, Gerontology, Psychology or Consulting preferred. Two (2) – Three (3) years experience in social service delivery with older adults required; Some management experience preferred.
Demonstrated working knowledge of community services in the region; particular knowledge of services that are provided for older adults; knowledge of aging process, elder services, disability services, drug and alcohol abuse, and mental health issues; knowledge of eligibility for and procedures of Federal and state entitlement programs; ability to communicate effectively and respond to questions from clients, employees, families and the general public; demonstrated ability to advocate, organize, problem-solve and provide results for the older adults served. Preferred training in the aging process, elder services, disability services.
