Coordinates personal and health related care of participants and conducts participants activities of the Adult Day Center in compliance with all applicable directives, including but not limited to all current and thereafter amended federal, state and local standards, guidelines and regulations required under this position.

Performs a nursing assessment and care plan, administers and monitors medications, disease screening, health education, rehabilitation therapy and counseling.

Assists with personal care of participants. This shall include toileting, changing soiled clothing, personal hygiene care as needed, assisting in ambulation, cueing clients to independently meet their own needs, monitoring food and fluid intake and all other areas of personal care services.

Maintain participant's records according to regulations and established policies and procedures. Notes and reports any changes in attitude, health, or needs of participants.

Attend educational programs relevant to job responsibilities and in accordance with LPN licensure requirements.

Participates in program planning.

Conducts program activities designed to meet, in accordance with the comprehensive assessment, the interests and physical, mental, and psychosocial well-being of each participant.

Sets up and operates equipment; leads games, singing, discussions and other programmed activities; conducts exercise activities, and interacts with participants on an individual basis.

Processes intake referrals, assessments of participants and admissions, maintains participants individual charts.

Works with volunteers assigned to specific projects.

Performs assigned duties to keep the Center safe, clean, and orderly.

Qualifications:Kentucky LPN license, first aid, and CPR certified, two years experience working with older adults, a self-motivated worker with ability to manage crisis situation.
