Full Time (32 – 40 hours per week) – 7 am – 3 pm shift

Essential Duties:

Facilitate accurate and effective resident assessment practices in the facility.

Coordinate, schedule and transmit all MDS assessments.

Responsible for learning the regulatory basis for the RAI process and maintaining familiarity with implementation of its components.

Provides documentation of a comprehensive resident assessment using components of the RAI framework including the Minimum Data Set (MDS) and Resident Assessment Protocols (RAPs).

Assesses the resident’s physical and mental status at the initial admission, at prescribed intervals, and whenever indicated, including times of significant change.

Develop implement and follow through with the Restorative Nursing Program (i.e., documentation forms, assessment process, evaluation/revision of planned services and schedule).

Direct the Interdisciplinary Care Planning team, including scheduling and meetings.

Requirements: Must be able to accurately complete and transmit timely Minimum Data Sets and related forms; possess excellent assessment skills; flexible with schedule and duties; comply with and enforce facility Policy and Procedure and use Regulatory Guidelines, must be a team player.

Qualifications:Education/Experience/Skills: Must be a graduate of an accredited School of Nursing and be a current active Registered Nurse in Kentucky. Two years experience as a Registered Nurse required. One year MDS experience and /or MDS certification required. Knowledge of the RAI process and QA/CQI process is preferred.
